Whether you have a sick pet that needs a customized diet plan, or a healthy pet that you just want to provide the best nutrition to, we can help! If you have any questions about customized nutritional needs, supplements, or feeding practices., he will help guide you and your pet to make the best, educated decision that suits your family. This can all be done remotely, so your pet doesn’t have to stress about going to the vet!

Consultation Charges (payable via Zelle, Venmo, or Paypal during the appointment)

  • Initial consult: $265

  • Follow up/rechecks: $150

  • Homemade or commercial diet nutrition plan: $695 for new/first time plans. $450 for adjusted DS Vet Nutrition plans.

    • $150 if rotational/variety options for new DS Vet Nutrition homemade diets are needed, or minor adjustments to a DS Vet Nutrition formulation is needed (supplement adjustment, vegetable and fruit swap, etc.)

How To Start A Consultation

  1. Reach out to DS Vet Nutrition via email at dsu.dvm@gmail.com Include your name, your pet’s name, and what you are looking for.

  2. Have your veterinarian fill out and return the Nutrition referral form (see below) and have your vet’s office email your pet’s relevant medical records from at least the last 6 months (Complete blood count, chemistry, urinalysis, and others). Because DS Vet Nutrition is not able to see your pet in person, we needs to work through your pet’s veterinarians (their primary vet and any specialist that they see) who has an established veterinarian-client-patient-relationship (VCPR) with you and your pet. Not only is this a legal requirement in most states, it allows your pet’s entire health-care team to know about your pet’s nutrition plan. Please note that DS Vet Nutrition is only acting as a nutrition consultation service, and the case responsibilities will remain with your veterinarian who will out the referral form.

  3. Once DS Vet Nutrition has received the Nutrition referral form and your pet’s medical records, a remote consultation will be scheduled with you.
